Telehealth

We provide avenues for improved access to quality care using telehealth and case0based e-learning platforms to enhance disease management.  Our pilot activities focus on providing care for Epilepsy and HIV patients as well as empowering local community providers with the knowledge to properly manage these conditions.  Our network of US-based specialists serves s trainers and facilitators allowing the community providers to drive disease management.

 

RISE Telehealth Services allows for the use of innovative technological advancements to better equip health facilities with telehealth services through increased training on telehealth technology, increased access to specialty provider networks, as well as increased access to continuing education opportunities in specialty care.  These services utilize electronic information and telecommunication technologies to provide patients with increased access to specialists while reducing overall healthcare costs. 
 

Community Centered Care

Our community centered care delivery model focuses on promoting collaborative and coordinated health systems to improve health outcomes and promote access to well-coordinated care.  Through our pilot in Nigeria, community health teams made up of volunteer healthcare professionals and healthcare professional students, offer disease screening, care coordination, healthy living and wellness training for the community.  They connect community members to primary care services and champion community based prevention of chronic diseases.

Our interdisciplinary network of providers comprising medical specialists, nurses, pharmacists, physical therapists, medical laboratory scientists, optometrists and social workers collaborate with local primary care providers and existing community based resources to coordinate disease prevention, chronic disease management, transitions between healthcare providers and healthcare settings and case management for patients.
